Job Safety Analysis Template: Free JSA Form + How to Fill It Out

A blank JSA template is only useful if you know what belongs in each column. Here's the format, a filled-out example, and the mistakes that make a JSA worthless.

A usable job safety analysis template has three core columns: job steps in the order they happen, the specific hazard at each step, and the control for that hazard, plus a header for job title, date, and who conducted it. OSHA does not require a specific format; its own Publication 3071 offers a sample worksheet, not a mandatory form. What makes a JSA useful isn't the template, it's whether it was filled out from an actual walkthrough of the job or from memory at a desk.

Most "job safety analysis template" pages hand you a download link and stop there. That's the easy part. The template itself is three columns and a header; almost anyone can build one in five minutes. What actually determines whether a JSA is useful or just paperwork is what goes in those columns, and that's the part this post covers, with a filled-out example instead of just a blank grid.

Here's what those columns look like applied to a real task, replacing a roof-mounted HVAC unit, rather than left blank.

Notice the control column isn't the same phrase repeated six times. Each one is specific to what's actually happening at that step, which is the difference between a JSA that reflects real thinking and one filled out to check a box. Is There an Official OSHA Form?

No, and this trips people up. OSHA's Publication 3071 includes a sample worksheet as an illustration of the method, not a mandatory form you're required to file on. There's no equivalent to, say, the OSHA 300 log's fixed format. Any layout that captures the sequence of steps, the hazard at each one, and the control satisfies the underlying requirement: 1910.132(d)'s hazard assessment in general industry, or 1926.20(b)(2)'s inspection requirement in construction. The format is flexible. What isn't flexible is whether the content reflects an actual walkthrough of the job.

Common Mistakes That Make a JSA Worthless

  • Same control, every row. "Wear appropriate PPE" written six times down the controls column is the single most common failure. It means the hierarchy of controls, eliminate, engineer, change the procedure, PPE, was never applied, and PPE (the weakest control) became the default for everything.
  • Task-level hazards instead of step-level hazards. "Fall hazard" written once at the top of the sheet instead of tied to the specific step where the fall risk actually exists. It tells you a hazard exists somewhere in this job; it doesn't tell a new worker which step to be careful on.
  • Filled out from memory, not from watching the job. A JSA written at a desk based on what the task is supposed to involve misses whatever's actually different on this site: the ladder that's six inches shorter than spec, the tool that's been broken for two weeks and worked around.
  • No sign-off trail. A JSA that exists as a document but was never actually reviewed with the crew doing the work is a filing exercise, not a safety control. The value is in the crew knowing the hazard and control before they start, not in the paper existing somewhere.

Paper Template vs. Digital JSA

A paper template works. It also has a specific failure mode: it gets filled out at the end of the week, from memory, back at the office, because that's when there's finally time to sit down with the clipboard. A digital JSA completed on a phone during the actual walkthrough carries a timestamp that can't be backdated, and the crew's sign-off is attached the moment they see it, not reconstructed later. Neither format is required by OSHA. One is a lot harder to fake.
